Health Services

A registered nurse is present at all times during school hours. We welcome phone calls or visits to discuss any health concerns you may have about your child. Please keep all contact, emergency, and health information updated in the case your child becomes sick or needs medical attention during the school day. Please inform the Health Office of any acute or chronic health conditions of your child, so the proper arrangements can be made.

An updated immunization record is required for students to attend public school. Please submit an updated record when your child receives any immunizations. For more information about New Hampshire school immunization requirements, please click here. Please see below for more important information and forms.

Physical examinations must be completed for each student prior to enrollment into the public school system. Click here to find the form that may be completed by your physician as a record of their physical examination. Your physician may use their own physical examination form if they so desire. If you already have a form on file in the health office, that is sufficient unless there are changes from the previous exam. This means a new health form is not needed every year for every child. Please call the Health Office with any questions regarding this.

Click here - This form is kept in the health office for the school nurse use only. Please review, fill out and send back to school. Please see below for more important information and forms.

    Medications

Parents and guardians are required to deliver all medications to the Health Office. Under no circumstances shall students transport medications, prescription or non-prescription, to or from school. If your child requires administration of an over-the-counter medication such as Advil or cough medicine, please bring in a supply in the original container labeled with your child’s name. A permission form for that is required. All unused medications must be picked up by a parent or guardian at the end of the year or it will be disposed of. We do not carry cough medicine or decongestants in the Health Office. If you anticipate your child will need cough drops during the school day, please send in a bag of cough drops with a note from a parent or guardian.

A physician’s signature is required to administer prescription medications at school.
